Please arrive 20 minutes early for your first appointment to fill out the necessary paperwork which includes a thorough health history. If you prefer, you can download the forms and fill them out in advance to save time.

24 hour Cancellation Policy 

If you have to cancel or reschedule your appointment, please remember to give us a minimum of 24 hour notice to avoid the $35.00 cancellation charge.

Payment Methods

Full payment is due at the time of service. Payments by cash, Visa, MasterCard, and Discover are accepted.
